Alberta Affidavit of Personal Service

When you serve a legal document on someone in the Province of Alberta, you need to prove that the document was served by swearing an Affidavit of Service.

This form is an Affidavit of Personal Service, which would be used when the document was personally served on the party receiving them.
An Affidavit of Service is required in circumstances where service of documents is required by law, such as legal proceedings, landlord-tenant notices and evictions, repossessions, foreclosures, etc.
