Alberta Affidavit of Personal Service

When you serve a legal document on someone in the Province of Alberta, you need to prove that the document was served by swearing an Affidavit of Service.
  • This form is an Affidavit of Personal Service, which would be used when the document was personally served on the party receiving them.
  • An Affidavit of Service is required in circumstances where service of documents is required by law, such as legal proceedings, landlord-tenant notices and evictions, repossessions, foreclosures, etc.
