Pet Foster Care Agency Agreement

Do you run a pet foster care program? Use this ready-made template to prepare a Pet Foster Care Agency Agreement for a client, allowing you to place the client's pet into a foster home while the pet's owner is away for an extended period of time.
  • The owner's pet must be up to date on its vaccinations. The foster care provider must certify that any other pets in the home are also up to date on their vaccinations.
  • The owner authorizes the agency to implant a microchip or tattoo the pet for identification purposes, if this has not already been done.
  • The foster care provider must not alter the physical condition of the pet without the owner's written consent (including spaying or neutering).
  • The owner will reimburse the foster care provider for any out-of-pocket expenses over a certain amount each month.
  • The agreement will run for a specified period of time, after which the owner will have the option to renew it for a further period if necessary. The foster caregiver has the right to terminate the agreement upon written notice to the pet's owner, in which case the caregiver must return the pet to the agency.
  • The file includes a foster caregiver application, to be completed by all persons who want to become foster care providers.
This type of Agreement is well suited to situations where a pet's owner will be working out of the country or is stationed overseas with the military, and is absent for long periods of time.
